Hospitalisation may not be avoidable, but when it is time to go home, your carer and loved ones may need additional care and support.

 

At The Three Cord Care, we can work with your carer and loved ones to transition from hospital to home ensuring that you will continue to receive the right level of care out of the hospital setting including practical arrangements for travelling and taking care of your safety and wellbeing in your home, focusing on personal care.  Some of the tasks include arranging GP appointments, collecting prescriptions, dressing and undressing, putting you to bed and waking up and everything else that matters to you.
